Location: Remote (with up to 75% travel)
Employment Type: Full-Time
Salary: $72,000 - $80,000 / year

✅ Position Overview:
This remote position offers the opportunity to travel across the U.S. conducting federally guided surveys. As a Clinical Social Worker Health Care Facility Surveyor, you will assess regulatory compliance and quality of care in Medicare/Medicaid-certified facilities, contributing to patient advocacy, safety, and system improvement.

✅ Required Qualifications:
☑️ Master's degree in Social Work
☑️ Licensed Clinical Social Worker (LCSW)
☑️ Completion of CMS Long-Term Care Basic Training
☑️ SMQT certification (Surveyor Minimum Qualifications Test)
☑️ Minimum of 2 years of recent surveyor experience (certifying long-term care facilities)
☑️ Demonstrated independent decision-making skills
☑️ Strong organizational and priority-setting ability
☑️ Willing to engage in open dialogue and request support when needed
☑️ U.S. Citizenship (required)
☑️ Willing and able to travel up to 75% nationwide

✅ Key Responsibilities:
☑️ Conduct on-site surveys for compliance with federal long-term care facility regulations
☑️ Lead or participate in various survey types (certification, complaint, revisit, comparative)
☑️ Observe care, interview staff and residents, and review documentation
☑️ Identify deficiencies and clearly communicate findings during surveys and exit conferences
☑️ Evaluate corrective action plans and conduct follow-up visits
☑️ Support internal training programs and participate in relevant workgroups
