Summary

We are seeking a compassionate and motivated In Home/Community Support Specialist to provide individualized support that promotes independence, skill-building, and community inclusion for participants. In this role, youll work 1:1 with individuals in their homes and communities to help them develop daily living skills, build relationships, and achieve the goals outlined in their Individual Support Plan (ISP).

The In Home/Community Support Specialists responsibilities are:

  • Support daily living skills, independence, and community engagement.
  • Assist with goal-driven activities and personal growth.
  • Complete required documentation (daily notes, progress reports).
  • Provide personal care as needed (toileting, mobility, feeding).
  • Maintain professional, respectful communication with individuals and families.
  • Follow all safety, training, and agency policies.

Position Requirements:

High school diploma or GED. Minimum of one-year experience working with people with disabilities, plus experience supporting adults in their home, adult day program or adult recreational setting preferred.
